Consent forms signed on the tablet, paper-free
The client reviews and signs with their finger. Zenivet stores the signature together with a hash of the document: if anyone changed the text, it would not match.
Consent forms are generated from templates with the client’s and the patient’s details already filled in: surgery, anaesthesia, euthanasia or high-risk treatments. The client reads it with you and signs on the screen.
On signing, the signature image and a hash of the text are stored, along with the signer, their tax ID, the date and the signature. The signed PDF stays in the patient’s record and cannot be modified.

A signed consent is not modified: it is cancelled with a reason and another one is signed.

Every area of the screen has a purpose. This is how it is organised:

How it works, step by step

New consent
From the patient’s Consent forms tab, choosing the type.

You review it together
The text already carries the client’s and the pet’s details.

Sign on the tablet
The client signs in the box and “Sign” is pressed.

It is saved
The signed PDF, with its hash, in the patient’s record.
Tip: Templates are edited in Settings → Document templates, with variables such as the pet’s name.
